Final report which outlines the development, implementation and performance assessment of HWPCP Community Grant Pharmacy Waiver Program
This publication captures some exceptional examples of case studies of PCP work across Victoria. The case studies have been presented in line with the Victorian Public Health and Wellbeing Plan 2016-19 and key priority areas.
Whittlesea Community Connections (WCC) in partnership with Cabrini Asylum Seeker and Refugee Health Hub established a pharmacy waiver program trial from WCC’s Epping office enabling people seeking asylum to access free prescribed medication and referrals.
Due to the growing demand in Melbourne’s west, local services began reviewing their service provision to children with developmental delays. Partners of HealthWest identified the need for a collaborative approach to this service review, to ensure local redesign initiatives were coordinated and in line with regional needs. In response, the Pathways for Children with Developmental Delays (PCDD) project was created for core service providers and other relevant stakeholders to work in partnership. The work would support a clearer and more streamlined service system that meets the growing demands for services for pre school-aged children with developmental delays.
