Supporting Patients’ Recovery (Central Hume PCP)
Central Hume PCP sourced and organised the delivery of self management training in the Ovens Murray and Goulburn areas based on evidence of specific training needs identified across 2016 to December 2017. They supported relevant stakeholders in using self management support as a core skillset when working with people who are living with chronic and long-term health conditions and promoted the training to Primary Health organisations that provide Community Health services or are PCP members in Ovens Murray and Goulburn areas.
Early Interventions In Family Violence (INW PCP, North East Healthy Communities, Health West Partnerships & Hume Whittlesea PCP)
A needs assessment conducted by local Primary Care Partnerships/PCPs in April 2014 across Melbourne’s north and west metropolitan region revealed that a large number of frontline staff from health services were not well prepared to recognise the signs of family violence, due to a lack of policies and procedures in place and limited or no training. The Identifying and Responding to Family Violence Project is a 5 year regional project which aims to improve early intervention responses to family violence by building the capacity of health and community services to identify and respond appropriately.
Buloke Family Violence Prevention Project (Southern Mallee PCP)
The Buloke Family Violence Prevention Project was developed for the rural communities of the Buloke Shire; Sea Lake, Wycheproof, Charlton, Donald, Birchip, Berriwillock, Culgoa, Nullawil, Nandaly and Watchem. Between 2016 and 2017, the number of family violence incidents reported in the Buloke Shire increased by 11.6%. Recognising family violence as a serious health and wellbeing issue that affects the local community, the SMPCP Buloke Strategic Health and Wellbeing Partnership 2017 – 2021 Action Plan identified preventing family violence as a key priority area. Being an isolated rural community, it is important that Buloke community members have the opportunity to participate in primary prevention initiatives locally.
Respin Gambling Awareness Speakers Bureau (North East Healthy Communities)
Harm from gambling impacts not only people who gamble but also their loved ones, communities and workplaces. In addition, that harm extends beyond high-risk gamblers – Victorian studies have shown that low and moderate risk gambling accounts for 85% of gambling harm. North East Healthy Communities recognises that people who have overcome the effects of gambling harm have considerable expertise on the issue, which is of value to others. Many of these people want to share their story to lend hope and inspiration to others and bring the often hidden experience of harm from gambling into the open.
