Shifting to a Shared Measurement Approach (East Gippsland PCP)

East Gippsland faces numerous challenges to health and wellbeing, including high rates of family violence, alcohol-related harm, obesity, food insecurity, and poor mental health. The prevalence of these issues in East Gippsland is statistically significant compared to the rest of the Victoria, and their causes are complex and varied. East Gippsland Primary Care Partnership (EG PCP) felt there was a need to reorient the way in which we tackled these issues. Rather than just focusing on action to address them, EG PCP decided to increase the focus on identifying and measuring the changes we needed to make.

BLG Health Needs Analysis Implementation Plan (Bendigo Loddon PCP and Southern Mallee PCP)

2015 – 2022 | Bendigo Loddon | Loddon Mallee

A partnership across the Bendigo Loddon and Southern Mallee PCP boundaries was established by health services and local governments of the three Shires of Loddon, Buloke and Gannawarra, called the BLG Health Network, with a clear role of supporting health service planning and collaboration to improve the health system in response to the place-based needs and opportunities in Loddon, Buloke and Gannawarra Shires. The Loddon Gannawarra Health Needs Analysis was completed in December 2016, recommendations from which underpin the implementation plan project, which translates the identified health needs priorities into Action Plans for each priority across the three Shires.

Southern Mallee Primary Care Partnership Integrated Planning Process (Southern Mallee PCP)

2015 – 2022 | Southern Mallee | Loddon Mallee

Following the development of the Southern Mallee Primary Care Partnership (SMPCP) Strategic Plan 2017-2021 in 2016/2017, SMPCP began working with each of the three SMPCP Local Government Area Partnerships - SMPCP Buloke Strategic Health and Wellbeing Partnership (Buloke Partnership), Gannawarra Local Agency Meeting (GLAM) and SMPCP Swan Hill Health and Wellbeing Partnership (Swan Hill Partnership) - to develop a shared vision, purpose and plan for the next four years. The aim of the process was to have an integrated partnership plan for each partnership to collectively work from for the four year period.

Transforming Lives (Central Hume PCP)

2015 – 2022 | Central Hume | Goulburn Valley

The Central Hume PCP team developed the concept of health outreach services delivered at the Waminda Community House and helped with planning and research regarding identifying need in the community, worked to connect the relevant organisations at a local, state and interstate level to support the delivery of the services and linked outreach services to the Community House. Benalla community members who use the Waminda Community House health services say they attend more health care appointments than they ever have because of the relaxed neighbourhood centre atmosphere, connections to other health and support services and casual conversations with staff.

East Gippsland Drug And Alcohol Prevention Alliance (East Gippsland PCP)

2015 – 2022 | East Gippsland | Gippsland

From its inception in January 2015, the work of the East Gippsland Ice Prevention Working Group (now known as the Drug and Alcohol Prevention Alliance) has reflected the needs of the local community. In response to community concerns about the use of crystal methamphetamine, the group focussed on providing accurate information about use of the drug across East Gippsland. As a result of these information sessions, a group of committed community champions were identified to be involved in designing local solutions to local problems.
