Chris Thorne (Aboriginal Community Support Worker - Koolin Balit) talks about his recent experience where advance care planning would have been of very real benefit. In this video, Chris encourages everyone – young and old – to talk with their family and write down an advance care plan so people will know their wishes if they become seriously ill or injured and cannot speak for themselves.
The local footy club is so often the heart of a community in regional Australia, and the small town of Tatong in Victoria's North East is no exception. In 2013, the Tatong Football Netball Club held its second annual Dreamtime Round to celebrate its Aboriginal and Torres Straight Islander players and their contribution to the club and local community. This short documentary film, 'Tatong dreaming', explores why this round is so important to the club, its players and the local Indigenous community. Central Hume Primary Care Partnership has supported the round as a Closing the Health Gap initiative and commissioned this film.
The Breastfeeding App was developed in partnership to guide families around the support and services that are available to them and to help mothers have a positive breastfeeding experience.
The Fair Feed program enables young people (aged 15 to 25 years) who are homeless (or have limited access to healthy food/cooking facilities) to access a meal at a cafe in Wangaratta or Benalla - at a time and location that is their choice. This program was piloted in Wangaratta and the model has since been implemented in Benalla.
In 2017, the Dirrawarra Indigenous Network (DIN) commissioned a feasibility study into the establishment and sustainability of a Gathering Place to meet the needs of the local Aboriginal and Torres Strait Islander community in Wangaratta. The feasibility study was auspice by Central Hume Primary Care Partnership. The feasibility study concluded that a Gathering Place is both needed and viable, but that a staged capacity building model for governance, operations and investment support is required to realise this aspiration.
