The western suburbs of Melbourne are home to many refugees, asylum seekers and other migrants who may be especially vulnerable and sometimes have complex health care needs. A Seat at the Table (SATT) was developed as an innovative, early intervention for mental health project. The project aimed to: Improve the mental health among young people through increased participation in mental health services. Improve health literacy and referral pathways into mental health services so that everyone has the option of accessing a service for assistance if and when they need.

The Briefing Paper to Consultants to develop a Tender Process for Digital Readiness Framework and Sector Needs Analysis. The major objectives to this project are to establish a baseline for member organisations of Hume Whittlesea PCP in regards to their organisational digital healthcare; Undertake a current situational analysis of Government eHealth Policy and regulations, I/T and I/M innovation in eHealth, health informatics standards and interoperability standards; Provide each organisation with a report, comparing their organisation to average of all organisations, on eHealth readiness and digital maturity and provide a framework for strategic direction on digital healthcare to member organisations.

Implementation Guide - Improving Coordination of Diabetes Care: Partner agencies have agreed on minimum services a client should receive based on best practice. The “HWPCP Diabetes Care Pathways” outline the services that all clients with diabetes should have the opportunity to access, at different life or disease stages based on the level of complexity of their condition.
