These snapshots have been developed to provide an insight into the health and wellbeing of people across the Central Highlands Primary Care Partnership. These focus on the 10 priority areas of the Victorian Public Health and Wellbeing Plan 2019-2023 catchment.

2020 Grow Together Khmer Green Kit Translated Resource

2015 – 2022 | Central Highlands | Grampians

Khmer Instructions sheet: Grow Together was an initiative led by enliven in partnership with SMRC during the pandemic. Planter bags, seedlings, Seasol, potting mix, recipe books and other resources were distributed to over 200 families to encourage growing food at home. Green, yellow, red, purple and orange kits were produced, each with different seedlings and separate instruction sheets.

Make It Easy: A handbook for becoming a health literate organisation

2015 – 2022 | HealthWest | West Metro

This handbook is designed to support organisations to become health literate. The handbook provides:
• a simple definition of what it means to be a health literate organisation,
• an explanation of the five requirements for being a health literate organisation,
• a five-step approach to becoming a health literate organisation, and
• two tools to help you get there.
The development of this guide was based on previous work by Gippsland Primary Care Partnership and updated by HealthWest Partnership and Inner North West PCP after collaboration between professionals and consumers from Melbourne’s west and inner north.
