Victorian Primary Care Partnership

This Outcomes Report follows on from the Central Hume Pharmacy Project – Final Interim Report. Local community pharmacies and pharmacists are often the first point of call for clients as they are the health professionals who see clients most frequently. Pharmacists can contribute to monitoring of clients between GP and specialist appointments and should be seen as part of the whole primary health care team. As a result of this project, other work and initiatives undertaken by Central Hume Primary Care Partnership will utilise and further strengthen the pharmacy relationships developed which includes involving pharmacies where appropriate.
