“The INW PCP, together with its member agencies sought to improve current pathways of care for people with cardiovascular disease (CVD) whereby clients are seen by the right service, in the right setting, at the right time.The transition of cardiac care from acute and cardiac rehabilitation services to primary care and community based health and wellbeing programs was the area prioritised for system level improvement work, following a Client Journey Mapping Workshop in November 2014.”
