Preventive health activity aims to stop the disease or problem from occurring in the first place by removing or reducing factors that negatively impact health and wellbeing, and promoting protective factors. This approach targets whole populations rather than individuals, with a focus on addressing the social determinants of health. It enables people to increase control over their health by improving the conditions of their everyday lives, and has broader economic, social and environmental benefits than efforts which focus solely on intervention and treatment.
PCPs support partner organisations to implement prevention activities by building local partnerships to coordinate action and providing; leadership, advocacy, an understanding of best practice approaches to prevention and evaluation, and building capacity to work across key health priority areas. Our work is guided by the Victorian Public Health and Wellbeing Plan 2015-2019 and the associated Outcomes Framework.
