System integration focuses on how different parts of health, social and community service systems can work together to provide people with consistent, coordinated, and streamlined care. It aims to create a more holistic system of care that is centred on people’s health and social needs to ensure they get the right supports, in the right place, at the right time. An integrated service system provides increased opportunities for early intervention and improved health outcomes.

PCPs facilitate system integration by bringing together local services to collaboratively plan and coordinate their activity so that the community experiences a health system that works together. Underpinning this work are the following key principles:

  • Social model of health and social model of disability
  • Competent staff
  • Duty of care
  • Protection of consumer information
  • Engagement with a broad range of service sectors
